Authorization to provide basic first aid to my child(ren) *

I authorize Crosspoint Staff, Workers, or Safety Personnel to provide basic first aid to my child(ren) in the event of an injury or medical emergency - while the staff attempts to notify me. I understand that the first aid administered may include, but is not limited to, cleaning and bandaging minor cuts and scrapes, applying ice packs, and providing over-the-counter medications such as antiseptic ointments, as deemed necessary and appropriate.
I release Crosspoint Staff, Workers, or Safety Personnel from any liability related to the administration of first aid or emergency medical care.
